Employers, use this spreadsheet to provide in an electronic format the required information Discovery Benefits needs for qualified beneficiaries who experienced a qualifying event September 1, 2008 or later up to the effective date Discovery Benefits began providing COBRA administrative services for your company. This would be used in place of completing the paper form for each qualified beneficiary.
