How do I request reimbursement for mileage?
You will need to calculate the mileage on the actual bill/receipt detail. Discovery Benefits requires that you show the number of miles driven times $0.24 (2009 rate) = total mileage reimbursement requested. Include the name of the provider visited (medical facility and/or pharmacy) on the reimbursement request form.
